To get it done fast, albeit at cost of quality: with a tablet, use the clone tool to keep the texture and erase any unsightly marks or bumps. For more airbrushed feel, just sample the color of the skin and pass over the skin at low opacity and softest edge setting, and make several passes as needed.

There are tons of different methods. Besides the fact that most of the people you see in cover shoots have image consultants to advise them and the money to act on that advice, everybody involved in the process is experienced and well-equipped. The photographers generally use high-end digital SLR cameras and work with an experienced graphic designer/artist. The artist will generally have Photoshop, with which he or she will smooth over the rough details while making the changes seem natural. This is the area where average come closest to matching those pictures. As long as you have a reasonable image editor (GIMP, Paint.NET, Photoshop Elements, Corel, etc.), there are tutorials at sites like Tutcast.com that you can follow to make your digital photos look close to what you want.
Hope this helps.
